Before you begin

Use this form to provide the supplementary information required when applying for a place at St Augustine’s CE High School.

• Your child’s birth certificate
• A baptism or dedication certificate, where applicable
• One recent proof of your home address
• Your faith leader’s email address, if submitting a clergy reference
